SUPPORT COORDINATION
Let's start right for you
Coordinating the right supports & service for your needs and preferences can be challenging. Often, a conflicted provider will suggest services that they themselves are well placed to deliver - as opposed to focusing on what you need.
Not with Amini Home and Community Care. We focus on you and if we are not best placed to support your needs from a service delivery perspective, we will work with you to identify the best provider through our extensive networks.
We are here to help. Amini Home and Community Care Care can assist you in a variety of tasks, which include engaging and managing your multidisciplinary care team, budgeting, planning for your future needs and making decisions on which supports are best for you.
We can support you to:
Understand your NDIS plan
Negotiate with providers about what they offer you and how much their services will cost out of your plan
Connect with community, mainstream and other government services
Plan ahead and prepare for your plan review
Get the most out of your funded supports
Navigate any issues with providers and advocate on your behalf
Ensure that you personally have a "direct line" to all of your providers

PERSONAL CARE
Our care team would love to support you with your daily personal needs - be it getting up and getting dressed for the day through to showering and administration of medications
HOME MAINTENANCE
From gardeners through to carpenters, general handymen and third party product providers , we have got you covered to ensure that you home is well maintained and functionally appropriate
DOMESTIC SUPPORT
Our dedicated team of caters, cooks and cleaners enable you to maintain a high quality of life in your own home. Leave the shopping to us - but at your direction of course!
RESPITE
Sometimes your family and broader support network need a break and some time by themselves to ensure that they remain positive and engaged in your support for the long-term. Our team can seamlessly slot in to filling all of their roles in your home at short notice. We are available 24/7, including peak holiday periods
TRANSPORT
Getting our and about is so important. We have a range of modified vehicles that are tailor-made for your individual needs and a team of drivers who would love to get to know you. Shopping, community activities or medial appointments - we have you covered
ACTIVITIES
We can facilitate great outings and activities for you that help you achieve your goals. Outdoors, Indoors, by yourself or with others. It's your call. We do however reserve the right to challenge you a little though as we want you to get the absolute most out of every single day!
SOCIALISATION
Whether it be catching up with friends, colleagues or attending interest groups - our team can support you as much or as little as is needed
CLINICAL CARE
Our wonderful team of Registered Nurses are with you every step of the way on the clinical aspects of your journey. Be it providing care or intensive clinical supports, through to interfacing with your GP, Pain Specialist, Therapist of Pharmacist - our experienced nurses are with you all of the way
ALLIED HEALTH THERAPY
All of our Allied Health team have worked in the sector for many years and are passionate about supporting people living with a disability to live their very best life.

PHYSIOTHERAPY
Our Physiotherapists support you to improve, strength, movement and flexibility as well as, in conjunction with our Exercise Physiologist keep you fit
OCCUPATIONAL THERAPY
Our Occupational Therapists, as well as completing any required functional assessments will work with you help develop a wide range of communication, motor, cognitive, play and sensory processing skills relevant to daily living
PSYCHOLOGY
We understand that your journey is not always easy and that it is sometimes good to talk. Our Psychologists walk with you and also engage with the broader team our involved in caring for your mental health
SPEECH THERAPY
Our Speech Therapist and Dietician work with you to develop a program to strengthen both your communication skills and to maximise your enjoyment of your diet
